They are starved because they either don't have the insulin that allows the cells to accept the sugar or the insulin they DO have doesn't work efficiently. When this happens, the glucose stays in the blood and is not absorbed by the cells to be used as energy.

Insulin is the key that opens the cells to accept the glucose.
Without insulin or without being able to use insulin, it just floats around in our blood and then comes out in our urine leaving our cells starving for glucose.
