Normally a "Snellen chart."
Though it doesn't ALWAYS start with an "E": that would be a bit of a give-away.

There are charts that consist of NOTHING BUT "E's"

A "Tumbling E" or "Illiterate E " chart has the letter pointing a variety of directions, and just requires the person to point in th edirection of the fingers of the E . It's useful with speech difficulties and illiteracy (including small children)

It's also possible to have this sort of chart where the letters don't get any smaller. They get paler, more faded out.
This is a test for contrast sensitivity, which can be markedly affected even when a normal high-contrast chart shows only minor or no problems.
(wearing soft contact lenses, early cataract, post-lasik surgery... all may sometimes show contrast sensitivity loss)

Landolt-C charts are similar, with nothing but differently oriented "C"'s, the object being to spot the gap in the ring.

The E or the chart? The chart is called a Snellen Eye Chart. The E is just the letter E.

The chart is called the Snellen chart.
It has letters of various sizes. "E" is just one of the letters on the chart. It so happens that the generic form of the chart has "E" at the very top the biggest letter

The last time I had my eyes checked I called it an "E"
