yes they should the policies should contain info on the allowed abbreviations. who can use them, in what context and where in the paitients notes

There are abbreviations that are not acceptable across the states. JAHCO (the commission that keeps hospitals in compliance) makes sure of this. I have worked at 2 hospitals and both have a site that gives you the unacceptalbe abbreviations.

most places already have rules and JAHCO has some strict requirements already in place for the use of abbreviations.
