Even in a monogamous relationship one might still need to get tested if they have any suspicions of infidelity on their partners part. Many people enter supposedly monogamous relationships infection free and end up getting an STD from their partner. With that said, I would say that if you are involved in a sexual relationship an HIV test should be included in your annual physical exam, just like when women have their annual gyn visit, in addition to the pap a chlamydia and gonorrhea testing is included. If you are not sexually active and you dont use IV drugs or work in an environment where you might contact it, then there is no need for an annual HIV test.

no, testing everyone annually would be astronomical in terms of cost, it simply isn't practical, and if someone is in a monogamous relationship or sexually inactive there really is very little risk so why go to all that expense? Not to mention the social stigma that goes along with being tested, regardless of the result. Being sexually active, as long as you are monogamous is not a risk for std. Being promiscuous and sleeping around, well yeah, that will give you a disease sooner or later.
