Toast on cheese if its one slice of toast topped with cheese.
Toast and cheese would work for that too I guess; but technically it'd mean toast and a bit of cheese on the side (eaten together; but not topped)
Toasted cheese is not at all the right expression as it implies you toasted the cheese alone (like you would cheese crisps or cheese sticks)

Grilled cheese though is the yummiest by far since you butter and grill the bread to "toast" it and the cheese always melts to perfection...mmmmm!

Welsh Rabbit in my house.
I lightly toast thick bread.
Grate a block of cheese add a tiny bit of mustard and a drop of Worcester sauce, a big spoon full of milk stir and pile onto the toast right up to the edges then grill under a very hot grill until it is brown. You can put a slice of tomato on top as well if you like.
Best in the world my children say.
