I'm doing a little project for someone I love, being jew is very important for this person, so I want to show I care.

It depends on who you are writing it to.

תמיד בלב שלי
(tameed beh-lev sheli)
The above part (forever in my heart) is the same. But the I love you changes depending on who is saying it and to who.

For a boy to a girl:

אני אוהב אותךְ
(ah-nee oh-hev otach)
For a girl to a boy:

אני אוהבת אותךָ
(ah-nee oh-hev-et otcha)

Lady Suri is right. It depends. If a boy is saying it to
a girl, you can say.
לעולם
בלבי אני אוהב אותךְ
L'olam b'libi ani ohev otach.

If a girl is speaking to a boy you can say
L'olam b'libi ani ohevet otchah.
לעולם בלבי
אני אוהבת אותך

Note: בלבי
is a shortened form of
בלב שלי
in my heart.
Also תמיד means "always". I much prefer לעולם,
which has more the meaning of forever.
Please forgive the crazy spacing here.
Blame it on the spacing of this medium!
