You go to the main administration building and ask for a replacement. This can be done by a clerk or yoeman (in the Nvay). There is a process and procedure for reporting the loss of an ID. The ID, if found, is supposed to be mailed to the Department of Defense or the proper branch of the military, but I wouldn't hoild my breath waiting for it.

Where is your husband deployed? you may not be able to get one unless you have a special poa, if he is deployed when i was in BCT they took my dependent id card from me ( I was in the Guard) soon as i came home from AIT; because my husband is Active duty and was deployed he had a special POA for me to get my id card back....I had to make an appt at the id card section present my POA in order to get a new one...
You may want to call the id card section and ask if your husband is deployed, if he isn't deployed and your still married he should be able to go up there with you to get a replacement, I heard they give Counseling statements to the service members if spouses loose their Id cards, I dont know of anyone this has happen to,

Go to the nearest military base. If there is no base close to your home, try the nearest reserve center or national guard armory. There should be a permanent staff that can get you a new ID card.

Go to the military ID card office on your base, wait in line, get a new one.
Also, if you've been to that doctor before, they shoulnd't need your ID card because they'd already have it on file.
