I can't speak for other city's but, for my local police department the physical agility test was prior to the medical physical. The physical agility test was soon after we passed the written exam. The physical agility test we jumped walls fences, ran an obstacle course, dummy drag, nothing too hard. the medical tests (stress test on treadmil, vision, hearing, lung, turn head to left cough) all came later in the hiring process.

You probably should call the department you want to work for or better yet, go in and talk with someone in the Recruiting Office and get a packet with all the information you will need. If you get it now, you will know what to prepare for.
One girl I worked with who was a civilian at our department wanted to become an officer and one thing she started doing 6 months before she applied for the job was to start running every day because she knew she would have to do that at the Academy every day while she was there. She started with 1/2 mile, went to a mile the next week and each week increased it until she was doing 5 or more miles a day.
She also started going to the gym and worked out with weights to build up the upper body and strengthen her legs and worked out on some of the other equipment just to build up her stamina. She made it and later retired as a detective in the Detective Bureau.
As far as your hearing and eyes, let the recruiter tell you if you go to your doctor and get a report or if they send you to one of theirs. Many times they have you go to theirs so they can be sure to get an impartial report.
Good luck ~ hope your dreams come true :)

The medical testing that you are referring to will be performed by a doctor during a pre hire physical and before any physical agility test.

USUALLY A TEST OF AGILITY IS:
SIT UPS =20 IN A MIN
PUSH UPS=35 IN A 2 MIN
RUN= SPRINTS OR A MILE OR WHAT EVER PUSH A CAR 20 YARDS= OUT OF INTERSECTION IF STALLED
CHIN UPS?
LIFT A DUMBBELL 20LBS CURLED EACH HAND 5 TIMES?
THE DEPT USUALLY SENDS OUT A SHEET EXPLAINING WHAT TO TEST FOR. THEY WILL EXPLAIN THE TESTING AND WHAT YOU NEED TO DO TO GET INTO SHAPE USUALLY A MONTH IN ADVANCE OR TWO.
CHECK WITH VARIOUS DEPARTMENTS AND ASK FOR THEIR REQUIREMENTS TO BECOME A POLICE OFFICER.

As far as the medical exam it depends on the department. My first department I had to have an EKG, run on a treadmill with a plastic hose in my mouth to measure what ever they measure, and much more.
My second department the medical was basically a annual exam. Turn and cough, reflexes, eye test, etc....
So it depends on the department by far. Eye exam will probably always be done. Usually you go to their doctor and they pay for it. Although one department I know of let you go to your own doctor for an exam, and you have to pay for it.
