It depends upon where you are moving. If you are moving a block away, you might want the babysitter again. If you are moving across the country, you probably should tell the baby sitter you no longer need her/him.

If you're just moving like 5-10 minutes away the babysitter might get offended.. But if you're moving to a completely different city, or area of a city, I'm sure the babysitter would understand.

Absolutely you tell them- with as much time in advance as possible.
I also like the idea of the card mentioned above. Very nice touch.
Maybe even a gift if the babysitter has been with you for some time and is close to the family.

No. Try to do all your packing and moving over a weekend and then have a hidden camera set up to record her reaction upon showing up at your empty house. Even better if rather than it being empty the new owners have already moved in as well.
Edit; Oh come on with the thumbs down. This is a silly common sense question.

if you want to be courteous and use etiquette then sure. She might want a recommendation from you for her next job. But it's not necessary, I mean, when doctors close down businesses do they write us a note to say they are closing? I recently had my vet move out of town and had I not phoned her on a whim to ask a question I'd never have known she was even moving (actually it was her last day in town).

Yes, you should give them a week or two notice. Some daycare centers have contracts and if you paid a deposit, usually one week of babysitting, you will not be able to get it back if you do not give any notice that you no longer need their services.
Also it is the nice thing to do.

Of course you tell them. I would give the babysitter a little x-tra cash to help her or him out a lil. She or he may of been depending on the cash and it would be a nice thing to do.

Yes, as when you register your child with the childminder you have to put your address and telephone number on a sheet incase your child ever gets sick or has an accident. If you are still intending to leave your child with the childminder just ask her/him for a new form but if your not, tell it to them straight from the start.

Would you appreciate a card/note/phone call from the babysitter if he/she were moving? (Probably yes).
