It depends on the drug.

Opiates like heroin or benzodiazepines like Valium or Ativan cause respiratory failure.

Stimulant-like drugs such as cocaine or amphetamines can cause strokes, brain hemorrhage and heart failure.

I agree with nice doc - it dependes on t he drug. Some affect your drive to breathe, some your blood chemicals, some your heart rate or rhythm, some may cause stroke or brain injury: just to name a few things
