Alright dude, this is what you gotta do. You have to have a condom session. Go out and buy an assortment of sizes and brands and try them all. Perhaps you've been using condoms too small. This experiment might introduce the ease of putting one on once you get the proper size. Or maybe you've just been too nervous or whatever to do it. Either way, with some practice, you'll get it. I suppose you could check the web for instructional videos like the one guy said.
Actually, if it turns out your condoms have been too small, you could always impress your lady with that.

That's frustrating! But condoms are important... and you're not the first guy to fumble with a rubber in the heat of the moment.
You might be putting it on wrong, or else it sounds like you're just nervous. Eithe rway- practice putting on a condom when you're alone... like before masturbating.
Check to make sure the condom fits. Most people can wear regular sized condoms, but some folks need larger sizes, like "Magnums".
Make sure you have a couple extra condoms before having sex- in case one breaks or you decide to do it again.
Keep in mind- you can always ask HER to help you put it on... that could be a sexy-fun part of the experience.
