You will have to check with the pizza place, some charge a delivery fee. Some was probably tax. Don't forget extra toppings cost extra. Just call them, you don't have to give your name.

I would start by calling the pizza place. If they have a delivery charge, they should have given you that information when you called to order the pizza, as they gave you the total due on delivery.
If there was no hidden charge, then the delivery guy could have been unscrupulous, and added his own tip to the ticket. Be aware that once you put your signature on the paper, it can be altered if you don't fill in all the blanks. You can stop this by writing on the check "no tip" and then be sure to put the total that was on the ticket.
They should never EVER have you sign a paper with one total and then charge you something else on your slip. That is illegal. You are signing to authorize a charge, and if it is not on the slip, then it is not authorized.
Call your credit card company and dispute the charge for the 3.00. They will get their fraud dept on the problem, and it could be bigger than just you.
No matter what.... delivery charge, tip, tire change, no matter what... if it is NOT on the ticket you signed, then you should not EVER be charged for it. That is fraud.

This is strange situation. I deliver pizza part time in Indiana and I have never heard of this. Typically when you phone in an order with your credit card, you don't discuss the tip. When the driver brings your pizza he/she will have a slip for you to sign with a space for you to write in the tip. When the driver gets back to the store he adjusts the transaction to reflect the tip. None of the pizza places I have worked at have taken tips over the phone. The only time that I ever see a tip ahead of time is with internet orders. When a person places an order online they go ahead and enter their tip amount at checkout. This seems weird because the tip comes before any service has been rendered, but it is very common. In this case the driver can see his tip when he prints your receipt prior to delivery. The drivers can't see you tip until they check out the run, so they can't pick and choose between runs based on tips. However, a driver might check out 2 or 3 orders at the same time. he might be tempted to take the better tip order first. I try not to that. With internet orders there is no place on the receipt to write in a tip, so if you want to tip the driver personally it needs to be cash. I would call and speak to the manager. Ask if this is a common practice. Employee turnover at pizza places is very high so it could have just been a new (confused) employee. I agree that it would be rude to ask you how much tip you want to leave. BTW, I have never seen or heard of anyone spitting or messing with anyone's pizza. I'm sure its happended but not in my experience. Drivers do remember good tippers though and they tend to take their orders first.

When you sign the credit card receipt, there is a place to add a tip. If you didn't draw a line through it and put the total again at the bottom, the delivery person did it for you. Any added tip by the establishment would have been on the bill stating it was a mandatory gratuity.

It could have been a delivery fee or maybe the employee who ran the card is dyslexic and saw a 9 instead of a 6. Call the pizza place to ask.

There might have been a delivery fee. It should've been included in the total though.
My brother used to deliver pizza. When the gas prices went up, his store upped their delivery fee, and the drivers didn't get any of it!

The $3.00 was probably a delivery charge as tips are optional

generally tips are not included. could have been tax, delivery fee, a mistake, or are you absolutely sure you didn't write in the tip when you were signing the receipt?

Apparently, it was included - unless there was a delivery charge.
