In my opinion you should, take your mom to an ER, there is no harm in taking her while you wait for a second opinion. They will sort out any acute problem or advise you some medication, BUT I tell you, she will have to give up the cigarettes or else many complications can arise.
Hope it helps you out.

The ER is for health emergencies. If she's in distress, by all means go to the ER or call 911. However, if you're just wondering what to do, call her family doctor and ask.

Call your Mom's Doctor and explain what is happening. He knows you Mom's history and would be in the best position to let you know whether 911 is necessary or not.

Heart palpitations can be brought on by smoking.

Symptoms may include:

thumping, pounding, or racing sensation in your chest
fluttering sensation in your chest
feeling of irregular beating or skipped beats.

You should take her to the ER if,

palpitations that last a few hours.
they occur often.
she also has sweating, shortness of breath, lightheadedness, nausea, vomiting, or pain in the chest, arm, back, or jaw, which may be signs of an imminent heart attack.
