They need a base line, so the only way to get a base line is for you to fast. I had to take my Mother to a City Hospital over her high Cholesterol and she had to fast from 11pm, appointment 8am. So I drove her two and half hours, we arrived they took a blood test and then I treated her to a big fried breakfast. We came back at 10am to the Hospital where they took another blood test. The fasting one was higher than the non-fasting one and yet she ate so much cholesterol you would think it would be the other way around. Not every one is the same, some will burn off cholesterol faster than others and so on. However it is up to the MD, to try and figure out how your metabolism works, slow, fast, exercise, age, diet it all has a baring and prescribe accordingly. I think they tend to think young people probably have no breakfast or a cereal and middle age probably fried while older cereal. If you are a rare milk drinker or a OJ with breakfast, the list is endless. To try and rule out contributing factors they just want to see the fasting level and then what you eat is up to you and not them, so eat food high in cholesterol, is your choice.

Hi there !
It is just that some of the biochemical values show marked variations from time to time, depending upon our mood, diet etc . In these investigations, fasting blood sample will be cumpulsory.
But some of the investigations are not depending upon time to time variations and will reflect the changes over a longer period of time. In these sort of investigations, a fasting sample will not be required.
Hope this clears the confusion ?
Best wishes !

MD's typically dont want people to fast at all. If people fasted more often, they wouldn't need MD's, and MD's would be looking for work. """"wishful thinking""""
