The system insists that you initially complaint in writing to the practise complaints officer. This complaint should be acknowledged by return and answered in writing usually within 4 weeks. If you are not satisfied with the reply, you can then write to the local Trust's complaints officer. You need to outline your complaint and advise that you are unhappy with the reply you have received from the practise.
If you do not go though the procedure in the correct order, the trust will reject your complaint.

If your GP has really messed up, feel free to ignore DR FRANK, who is so entrenched in an outmoded system that he barely knows his aspect from his elbow. Complain immediately to your local health authority, copy your complaint to the British Medical Association and if you can afford it get a solicitor (if not try to get a TV programme on board)!
The medical profession have been too arrogant for too long and though I realise we are all human, if they don't want the responsibility then they shouldn't do a job that we all have to pay them for whether we use them or not. The only thing you have expected is that this person do their job and if they cannot they do not deserve to have that job.

your GP prescribes medications but is not responsible for the administration of the drugs. might be the pharmacist you need to talk to?
