It depends upon whether you are having the baby in your own country, or in a foreign country.
If you are having it at home, then you can either do:
a) Add the child onto your passport, and the child can travel with you on that passport until (usually, depending on nationality) five years of age.
b) Wait for the birth certificate and then apply for a separate passport.

If you are having the child in a foreign country, go to the Embassy or Consulate of your country and register the child and at the same time put the child on your passport. You can later make a seperate passport when you have both Birth certificates of your country, and of where the child was born (in case of dual citizenship).

Not many countries will let you get a passport for a child if you don't hold the birth certificate (this is to prevent international child abuduction).

If you're American it can vary from which state you're in (you may be able to ask for a priority processing which can cost extra). And also depending on which nationality you hold you may be add your child's name to your passport (which is pretty rare now).

You order the birth certificate in the hospital. It comes within a couple of weeks. You should not, however, travel with a young baby...especially out of the country.

You can't and should not travel with a newborn until they are at least 6 weeks old. That is plenty of time to get your ducks in a row and get needed documents.
