I had a passport years ago but lost it a while back. I haven't needed one in all this time, until now. The application asks if you have been issued one and if so to put the passport number. The problem is that I haven't seen the thing in years (it's expired by now anyway) so I don't have this information.

Do I say that I haven't had one before?
Do I say that I have and leave the passport number blank?

What should I do?

Yes, you have had a passport issued, so answer the question "yes." Under number, put that you don't know; there may be a section for other information where you can explain that the passport was lost and expired.

Add: Actually, on second thought, maybe you should be filling out a "lost passport" application rather than an application for a new one. However, you'll need to apply for it in person in that case.

Go to your local Post Office. Tell them your situation and ask them for their help in filing the correct forms to "replace" your
"lost" passport. Make sure to bring along about $300 because the fees almost doubled earlier this year. Bring double that amt if you need express service. But you should know that they have been so backed up all year that they have been unable to provide express service. Be prepared
to hurry up and wait

Always photocopy your passport for your files.
