Not just 4x4s..can be found on 4x2s and on horses that stand at least 14 hands high (ok, loved all the Cowboy lasso descripts and had to put my own in here)
But it used to mean the second to highest level (But since 1997 model year, pretty much highest level) in the Ford F-series of trucks
As others described.. all the bells and whistles..stuff you won't find on the lower models such as heated and sometimes cooled leather seats.
Any option packages above this, are based on the lariat however now, and are usually just seat material changes (such as the Harley Davidson and King ranch editions)

The word actually means a rope used by cowboys to lasso cows and steers.
As for as Ford is concerned, it is a trim level. It is one of the higher trim levels that Ford uses for the F series trucks. It has all the bells and whistles that are available, or at least most of them, the others are still options.

It is Ford Motor Companies designation on the F-150 that it is the top trim level truck.
They come standard with leather, climate control etc. etc.
