Some people know what they see and hear isn't real. Some people don't. i follow the philosophy that if he doesn't threaten to hurt someone or himself then why does it matter what they say?
Whenever I'm in that situation I sit and talk to people. i step inside their reality for a little bit and ask some questions. Don't call them crazy or stupid. Nobody likes to be disrespected. Everyone just wants to feel like someone is listening.

A person who is mentally ill and not being treated with medication, therapy or counseling may indeed persist in their insanity. They have lost touch with reality and believe it is reality.
A mentally ill person who has had help, on meds, therapy, counseling, etc., is aware that those ideas are crazy and does not put stock in them. They are in recovery.
It is like alcoholism. The recovery is for life.
I know of mentally ill people who have gone off their meds and stopped getting help and they have relapsed.
At the same time, even those who faithfully take their meds and do their therapy and counseling will have occasional bad times where they struggle with their behavior. It is called an episode.

How do you know for sure that someone is crazy? Maybe they do know something you don't! Most truly crazy people don't know it. Those are the people that try too hard to fit the definition of normal, and believe the experts and the authorities without question. They scare me!

Speaking as someone who is "crazy", yes most times in at least the more lucid moments, "crazy" people are well aware that there is something wrong with them.
Now if they have had or are having a psychotic break? Nope. Get em to the closest emergency room.

No, they don't know they're crazy. They usually believe they are the sane ones. And no, you can't really reason with them either... because they have everything already Rationalized within their own mind and anything you say contrary to that will only anger them.

If they did know they were crazy then they are not crazy. Vice Versa.
Its the classic Heller debacle in the book "Catch 22" and hence that is where we get the term "catch 22".

From my own personal experience, no. That's what makes them crazy--the inability to understand that their thought processes are out of whack.
Seems to be an example of a general Law of Denial. Truly stupid people NEVER admit they're stupid, while smart people usually admit their intellectual shortcomings. Incompetent people think they're competent, while alcoholics are incapable of really admitting to themselves that they're alcholics. Human nature, I guess...

Depends on the severity. Very evere cases of delusions and schizophrenia have complely lost touch with reality, they actually beleive 100% in the delusion. Less extreme sufferers know that what they are seeing isn't real, and in some ways, that is much worse.

No! In their world every thing seems real and normal. They see other people are the ones with the problem. They are also happier in their world because they are use to that world. They can be awesome or scary!
