Hi
You should have a cbc done which is a blood panel, That will let you know if the organs like the kidneys are healthy.
If your cat has had vaccinations 1x, I would NOT get boosters or additional vaccinations. They are not proven to extend the life of the originals and in fact may weaken them. Also, the amount of problems caused by vaccinations is a dirty little secret that the public really isn't aware of. Tell your vet that vaccinations were given last year (sorry about the lying) if he asks to avoid confrontation

They will check for general health issues, bone strength, condition of fur, and other medical problem that can occur from old age.

Unless you cant has a problem dont get any shots for your cat, vets generally give animals unnecessary jabs just to add an extra $50 on the vet bills

I'm sorry, pardon my ignorance, but why would you ask a public forum as to what your vet should check for? Do you not trust your veterinarian to make those decisions? If you don't, than you should look for a different professional. Please remember that veterinarians have doctorate degrees and went to college 8 years (at least) for their profession. And, since your cat is 12, someone must be looking out for Milo and doing a pretty good job (and that includes you). Good luck!

The vet will do a senior check up. He will probably suggest a complete blood panel. If the cat goes outside, there are definitely shots/vaccinations he should be getting every year.

Yep a shot and maybe an offer to clean teeth. I'm sure it will be around 100 dollars. When our cat was 19 our vet wanted to do all kinds of testing and teeth cleaning with anesthesia but we decided natural was best and declined all that ;so she lasted one more year which was good ,as she had a very luxurious life .

The vet will probably check his teeth, eyes, ears and listen to his heart and lungs. He will probably do blood work to make sure his liver, kidneys and other organs are working well. Most vets will offer booster shots. Since your cat goes outside, I would allow the vet to give boosters.

At his age, he can still live a while. But it's good to start doing a checkup every six months instead of every year, that way you can catch something early. As for shots, as long as he got them when he is young then I would not overdo it, as too many shots cause cancer. But then since you keep him outside sometimes, I am not sure. It's safer to keep inside, especially as they get a little older.

He will give him a physical, including a blood test to see if he has any contagious diseases that can be passed from other cats.

Outdoor cats should have shots, at least feline leukemia and FIV vaccine.

If your cat is outdoors at all, a yearly checkup is a must.

12 yrs old isnt actually too old for cats.. I know a lot of cats who live like 18-25 yrs old... If yours has no problems, or symptoms they might not check for anything weird, but will do routine check ups and maybe blood work.
