My pain Dr put me on tegretol xr to start in a few days and I am reading all the warnings and freaking out.... if I decide not to take it do you think he'd find out by the random drug testing I get done at his office? I am on narcotics as well and get tested every so often. Does he just check for the narcotics or will he check the urine for the Tegretol as well?

First, you need to contact your doctor to discuss your reservations regarding the Tegretol XR. Normally Tegretol levels are checked using blood levels, but they can be obtained and will note the absence of the drug. If you are concerned about the drug and the warnings, discuss this with the doctor, many of the warnings which are provided on the pharmacy handouts and websites are quite alarming if you don't expect them. However, the medications are quite effective and usually can be very beneficial. After discussing these concerns, if you still do not want to take the medication, tell the doctor, and allow them to offer alternative therapies. Don't just suffer needlessly because you have not been fully advised about all the issues related to the medication.

Most of the anti-epileptic medications used for pain related to neuropathy, or headaches or used for seizures (and also for other treatments) have significant warnings, but usually can be managed if you understand what to watch for.

Just take the time to discuss the issue, don't avoid the medications because you are concerned about the warnings.

Important information

You should not take Tegretol if you have a history of bone marrow suppression, or if you are allergic to carbamazepine or to an antidepressant such as amitriptyline, desipramine, doxepin, imipramine, or nortriptyline.
TELL YOUR DOCTOR ABOUT ALL OTHER MEDICINES YOU USE. Some drugs can raise or lower your blood levels of Tegretol, which may cause side effects or make carbamazepine less effective. Carbamazepine can also affect blood levels of certain other drugs, making them less effective or increasing side effects.
Tegretol may cause serious blood problems or a life-threatening skin rash. Call your doctor if you have a fever, unusual weakness, bleeding, or a skin rash (no matter how mild).
Do not stop taking Tegretol without asking your doctor first, even if you feel fine.
If you are pregnant, do not start or stop taking Tegretol without your doctor's advice.

the side effects you have read about are fixable and will resolve if you stop the meds. you're doc won't test for tegretol in your blood or urine. you should probably go back and see him though because the doses of the other meds you take may be lower if he thinks you are on this one too.
