Medications all have different ways of being absorbed into your bloodstream, and if you take them with food when you are supposed to take them on an empty stomach, it might take longer for them to disseminate and will only be partly effective. It's along the same reasons that some pills are supposed to be taken at the same time every day-- because their make-up means they will stay in your system for X period of time, so if you take it sooner or later than you are supposed to, then you are having too much, or letting it out of your system completely.

Hope this helps!

some medications interact with the stomach acids in your stomach. Some medications MUST be taken on an empty stomach, like acid reducing drugs for stomach issues. Some also react poorly with some foods, like a few different antibiotics. Sometimes the food can render the drug useless. Getting sick also doesn't happen to everyone.

Medications to be taken on empty stomach are those which absorption are affected by foods or those which potency are affected by gastric juices. The presence of food stimulates gastric juices secretion so if the medicine is taken with food, it will be greatly affected - delayed absorption or decreased potency.

It has to do with how the particular medicine is absorbed into the body. Some medicines need food to mediate the uptake of the active ingredient, some don't.
