Could just be superficial skin infection, commonly called 'hotspot". Could be mange (sarcoptes or demodectic). Could be ringworm (not a worm, is a fungus). Could be allergy.
Has he been checked for mange? Ringworm? Bacterial skin infection? If not, he should have been.
He needs a shampoo that is anti-itch and calming -- there are lots of different types and I don't know what's available in your area. We carry a shampoo called "Relief" among many others. He may need antibiotics. Be absolutely certain that you're controlling fleas in environment and on him. Of course if your vet found ringworm and/or mange, he should be on treatment for those. Allergies are frustrating b/c can be so many things. Hypoallergenic diets are available in dry form at pet stores and at vet clinics.
You can try benadryl. Our dose is 1mg/lb of body weight (ie, a 50 lb dog can take 50 mgs of benadryl). Please call your vet if you want to try benadryl.

Good question. Allergies can be bad. I have heard what you are saying hundreds of times. Get the corn, wheat and soy out of the diet. These are the three most common allergens and around 70% of dogs are allergic to corn by the age of 6. Try this and if it does not competely inprove go to grain free. this is a bit more expansive but not any more than Hill's Crap Diet or Royal Canin. Some good choices for a GSD are Solid Gold Wolf King or Azmira either beef or lamb. If you can't find these go for wellness or merrick. If none of these are availible the last resort would be evo/ enova. The web sites are below so you can find them in your area.
If you are very ambitious try urban wolf or honist kitchen. These are prepare at home diets that are pre balanced for a complete diet. This is what my dog eats. Its the best food in the world and I have seen even cancer go into remision and fatty tumors disapear with urban wolf. Very cool stuf!

My dog has severe allergies (always itching, biting his skin, pulling out his hair, etc) and the best thing that we do is use a shampoo called Epi Soothe that we get from the vet, and bath him 2-3 times a week. Our dog only weights 20 lbs so that is not so bad, but with a German Shepard that might be a little harder. You might also want to try benedryl and slowly switching to a food that does not have corn as a main ingredient ( A lot of dogs are allergic to it)

About the only thing I could think of is some cortisone cream. But if you have something from the vet, it probably has some steroids, and maybe and antibiotic as well as an anti-fungal. It's probably better than what you can get off the shelf, so I would stick with it.
I wouldn't bathe the dog as that will only tend to dry the skin more, although you might be able to get a medicated shampoo from your vet.

Gilbert O'Sullivan - Alone Again (Naturally) ALWAYS makes me cry for several reasons. In one cinema, they always play this song before the movie starts and one day I went to the cinema with a group of people and it was very hard to keep the tears away fro them to say the least :P Also, Simon & Garfunkel - Bridge Over Troubled Water makes me teary-eyed, Art's voice is amazing and so is Paul's lyrics and melody :) Oh and there is a Russian composer, Borodin and he has this amazin track, it's called 'Polovtsian dances' and when the choir starts singing.. It's just breathtaking! I couldn't find the singing version on YouTube though, we had to listen to it in school and we had an amazing version of it!

It sounds like your dog has an omega 3 or 6 deficiency. My basset/beagle had very bad dandruff and dry skin and my wife created this oil mixture and all his symptoms are gone. Check it out:

Switch to a grain-free food. That is one of the biggest allergens for dogs and one of the easiest and cheapest to fix. Worth a try before trying any other method.

there are oatmeal shampoos you can buy for cats/dogs. Food makes a huge difference in a pets diet. It may cost more, but you will see results.

go to the drug store and buy a can of "Bag Balm". It comes in a small green can. Its a sauve and works wonders. I use it all skin problems, people and dogs.
