The cigarettes can be a factor in this. So can thyroid issues. You should see if what you are having are tachycardia arrythmias (weird nerve triggering issues) or PVCs (premature ventricular contractions). Tach can be a simple misfiring of the nerves that trigger the heart. While this is not usually life-threatening, it CAN cause a drop in blood pressure and subsequent fainting. Also, if it continues for too long a time, it can cause some damage. Often, this is caused by having more than one nerve leading to the heart and causing the mis-triggering (my friend had 3 of them!). Nowadays, there is a very simple, mostly non-invasive surgical procedure that can be used to sever the nerve at fault and leave just the one good nerve in place. Or, if the condition becomes extreme, medications or a pace-make can be used to control it. However, often this condition will simply disappear on it own (like mine eventually did!).

PVC's can be stress related. Again, they are not life-threatening, but should be looked into so that it can be determined that there is no underlying physical reason for them happening.

When I was hyperthyroid I had heart palpitations constantly, if I were you I would get my thyroid checked out.
It can cause many more symptoms and eventually lead to heart failure due to it working so hard.
