the donors and patients generally have to be ABO compatible however research is being done on kidney transplant with non compatible ABO grouping. However this would require increased amount of immunosuppression and plasmapheresis. the patient and donor must also share majority of the HLA and minor antigens. This and the same blood type will allow for greater chance of the patients body not rejecting the kidney. There is no longer a necessity for the patient and donor to be compatible.

Donating kidney and blood group change is no problem
First kidney can be donated and same blood can be get from
others during surgery. Skin matching is important.
