For Australian passports go to
http://www.passports.gov.au (main page)

child passport info - https://www.passports.gov.au/Web/Newppt/ApplyingU18.aspx

"An Australian citizen under 18 years of age who has never married is regarded as a child. A passport application for a child must be lodged by a parent or other person with a parental responsibility for the child. The person lodging the application should bring proof of their identity."

Not sure on prices but I got one for my baby last year and it cost near $200. Prices go up all the time .. typical of a government dept. (it does say $100 on the web though) your local post office will be able to give you the correct price to pay. You can pick up a passport application there also.
Make sure you have your orginal birth cert, as they don't accept copies.

If you haven't heard anything from your father and don't know how to contact him your mums signature should be enough. She would just have to sign a Declaration stating she has nothing to do with him and has no idea where he is.

is this for a US passport? You propose with you hand on your application? all your info would be ordinary and your mum and dad's info could be on the variety. If there is something incorrect they might actually help be responsive to long until now you get a passport.
