You could make paper airplaines and fly them.

Here is a site that shows you how to make some:

I have a four year old son and I can tell you what he likes to do but it really depends on your nephews personality.

Here are the things my son and I do:
go to the park or just explore outside
legos (I come to really love legos) and lincoln logs
watch tv (I don't usually actually participate in this and I limit it)
ride bikes
fishing
arts & crafts (color, paint, glue craft sticks, glitter, PLAY DOUGH!, etc.)
my son likes to build forts so we occasional scavenger out big boxes and cut doors and windows and decorate them (beware boxes can be very dirty)
my son also likes to freeze stuff in water so we look for things to do freezing experiments with (recently we've frozen a shirt, mushrooms from our neighbors yard, plastic army guys)
paper airplanes
build train tracks for our 3 year old son who's really into the trains themselves
If you are just visiting from out of town and want to go for the 'bigger stuff', you can take him to the movies (okay, well that's big for us anyway), take him to the zoo or an amusement park, go on a train ride (we just go locally for fun), go to a local airport and watch small planes land and take off (pick a weekend day for this one) or those hideous game places that I hate like "Fun Depot" if he's into that.

Good luck!

At that age just about any kind of playtime would entertain him, go to the park and play tag, do some arts and crafts, put on some music and have a dancing contest

draw with him maybe teach him how to write his name ( or at least make some of the letters ;p)
play catch with a plastic ball or with a stuffed animal but add the word super to the beggining of his name example teddy becomes super teddy and say super teddy to the rescue then chuck the bear across the room
try and build stuff most kids like blocks and stuff or if worse comes to worse throw on his favorite movie (or yours)
