Usually they require the child's full name to sign them up for school, but they will generally ask you what you want the child to be called. I had a similar issue. My daughter's name is Annastasia, but we have always called her Anna. They call her Anna at school, but when the teacher is teaching the kids to write their names, they have her write her full name as practice.

I would suggest using his given name on the application. If there is a section for comments write that your family calls him "Andy" at home and he preferes it to Andrew. You can always speak to the daycare provider as well. This should not cause a problem for you or your son.

You should put Andrew, and then it will either say on the form, or you can request directly to the teacher, that he be called Andy, overall. But you'll want his full official name on the enrollment form.

Any legal forms, and that includes school forms, require a person's full name. Just write "Andy" next to "Andrew"

I would fill it out Andrew and put Andy in parenthesis. My son goes by his initials and that is what I usually do. And be sure to tell them that he goes by Andy.

No, they usually want his legal name. However, I see nothing wrong with making a note that he goes by Andy.

You can. My daughter's name is Alexandria and we call her Lexi. Even in elementry school I use her nickname, except on papers like the medical forms. A lot of times there is a space to put a nickname or the name you prefer.

Wow, a lot of people are taking this enrollment form very seriously. It's not like he is applying for a mortgage or something. Just put Andy down and if they don't like it they can scratch it out and put his "real" name down instead. It ain't no big thing.
