If they are old enough to ask and understand the answer since she is in puberty she is. They are old enough to know the truth.

Absolutely, yes! Never be afraid to answer any of your childs questions when it comes to sex; how else are you going to have an influence over their choices in the coming years?
Answering the condom question:
Tell her it is used by the male during intercourse to stop unwanted pregnancy and sexually transmitted diseases - eg HIV. Explain that when people have reached the AGE OF CONSENT (that might help) they should ALWAYS use a condom as they don't know who their partner last slept with.

If shes hit puberty then yes, she should hear it from you (other than others), and you shoudlnt make a big deal about it. Im sure shes heard of sex, its hard to watch t.v or any movies these days without having it jammed down your throat, so just tell her that condoms help to stop pregnancy and stds, you might have to go into more detail about STDs but they need to know about them too... kids are having sex earlier and earlier these days, and whilst us parents shouldnt be teling our kids its ok to have sex, they should be informed of things so they know whats out there to protect them.

I think once a child is old enough to ask, its time to give the simplest answer possible without fanfare.
Maybe a simple answer of birth control would be enough, try that first...if she wants to know more then give the next simplest answer. Its never easy to answer these types of questions...but in this case honesty is the best policy.

i think that she is in puberty it is time to tell her what it is because if you dont she might get the wrong information from somewhere else. that way she has a clear understanding and it shows that in the future she can come to you for advice on other things when the time comes.

What's the alternative, say "no I can't tell you" and the child then goes to her friends for mis-construed information? I think the answer is obvious.
