The second answer is close to correct.
The term open-heart surgery generally refers to an operation in which the heart-lung machine is used to support the patient’s circulation while the surgeon opens and makes changes to the heart.
Coronary artery bypass graft (CABG, pronounced "cabbage") is usually performed as open-heart surgery, although in some cases it has been done by slowing the pulse and grafting the bypasses by making the stitches between heartbeats, eliminating the need for a heart-lung machine.
Open-heart surgery is also used for valve repair surgery and heart transplants.
