STIs are almost always spread by sex (vaginal, oral, or anal) or by some sort of sexual contact. There are a few other ways they can be spread like childbirth, but this is pretty rare.

Abstinence is the only thing that is truly 100% effective against the spread of STIs. Condoms definitely HELP prevent the spread of STIs, but they are never truly 100% effective, especially with STIs that can be spread by DIRECT skin to skin contact like genital herpes and HPV. A condom only covers a man's penis and not his entire genital area. So, just the rubbing back and forth of the genital areas during sex or foreplay is enough to spread either virus.

Bacterial STIs like gonorrhea, chlamydia, and syphilis can be treated and cured with antibiotics. Viral STIs like herpes, HIV/AIDS, hepatitis, and HPV can be treated, but NOT cured, with antiviral medication. There are no cures for viruses.

I got clinically diagnosed with the herpes simplex virus (type 2) about six yrs ago, when I was still attending college and had a dumb one-night stand.
