you need to see another doctor. Laugh all you want to but I am a psychic. Your wife needs medical attention very soon. There is an issue with her heart but also with her digestive system. Get a second opinion ASAP. Also the kidney may have something to do with the digestive tract problem but not with the chest pain. Just as a validation of my psychic abilities someone around you has an uncommon issue with their eye or eyes. Please see a doctor.

Doctors may SEEM like they're guessing... but the people on Yahoo! Answers really WILL be guessing.
Truth is that there are a billion things that can cause heart pain. The number one heart pain that turns out to be nothing and makes you feel stupid is good 'ol heart burn. Yes, heart burn can be so bad that it feels like a heart attack. I know I learned my lesson about eating Jalepenos after 8 p.m. Oh, yes.
Other reasons could be clogged arteries, high cholesterol, rapid changes in blood pressure, muscle spasms... theoretically, it could be anything. Often, the only time to be sure is to do exploratory surgery or wait for an autopsy. I doubt they want to do exploratory surgery at this stage... and NOBODY want the autopsy option, obviously.
Don't you ever watch Dr. G Medical Examiner? Discovery Health Channel is pretty much rad. It is very informative, too. Anyway, don't give up on the doctors. Be patient. They are unraveling a mystery. It takes time. I promise they are doing their best to figure it out and not trying to make you angry at them. Diagnosing involves a lot of guess work, ruling things out, and trial and error. I know how frustrating it is, but seriously, if it was as easy as figuring out you had the flu, then we wouldn't need doctors.
You also might want to try holistic therapy IN ADDITION to your traditional medicine. Accupuncture has been around for hundreds of years. It has helped a lot of people I know from every thing to back pain to fertility issues. Either way, it never hurt anything to go check it out and explore your options.

You may also want to have her lungs looked at. If she is having chest pain that isn't caused by her heart, there are other things is could be. Have an x-ray taken of her chest just to rule out any of those possibilities. I personally have never heard of chest pain caused by the kidneys. It usually is just the lower back that gets sore when the kidneys are having problems.

You answered your own question. And, I suspect you know that you did. Your wife has a bad dietary habit, obviously, and the reason she needed bypass 11 years ago is still with her. She had to be told by her doctors to change her life style, and obviously she didn't.
If she is having chest pains, it is her clogged arteries, OBVIOUSLY. If she wants to live, she will look hard at her life style and change what iskilling her. If she doesn't care whether she lives or dies, she will go on eating greasy, high preservative, fatty foods and eventually her clogged arteries will kill her.
The choice is hers. For you it is called TOUGH LOVE. Do you want her to live. If so, force her to do something about her life style. Stop sugar coating it

You can have referred pain from the kidneys into the chest and the arm area. I would not bat an eye at getting a second opinion. NOBODY can diagnose what is happening or not happening on this blog.

I would merely be guessing but sometimes when I really need to urinate badly, I feel like I have chest pains as well. That would be kidney related.

Clearly something is badly amiss here. Get another opinion. With her history, the doctors should be swarming all over her with their tests. If they are not, go find you some different doctors, before you end up losing her.

In my experience she has reached the the END ORGANS damage, please take her to a good hospital and make the rest of her life as happy as possible, be kind to her in words and deeds.
