experiencing bleeding after she had sex. She has no leasions or warts, no discharge no pain w/urination. We observed that she had mucopurulent exudate, we can also cause slight bleeding when a swab is pass through the cervix. She has been sexual active 4 two years w/one parter. He was treated 4 gonorrhea with fluroquinolone ( in Thailand). I was thinking that this girl does have gonorrhea because fluoroquinolone is resistant to N. gonorrhoeae????
What is the diagnosis and the causative agent?

In the US, N. gonorrheae can be cured with fluoroquinolone (and often is). But you said THAILAND. CDC states that there is a rise in antibiotic-resistant N. gonorrheae in Asia. CDC guidelines for treatment state that fluoroquinolones can be used (ciprofloxacin et al), but lists cefixime (a cephalosporin) first as a recommendation.

Seems to me that this girl's partner got the fluoroquinolone-resistant gonorrhea and brought it home to her. Her symptoms are consistent with gonococcal infection: no warts, no lesions. You often get dysuria with gonorrhea, but it's not necessary in all cases. The cervicitis and mucopurulent discharge is classic for gonorrhea, though. In this case, both people need to be treated with cephalosporins, and abstain from sexual contact until tests confirm they have both cleared the infection.

In other news, sex tourism is bad. It's shameful to the poor girls who get exploited. It sickens the men who buy into the sex industry. And it sickens the innocent partners of these men back home. I'm Thai, and I'm frustrated with the international perception that Thailand is a country full of easy and diseased women. Thai women are better than that, and if given the oppportunity and education, we can be doctors, teachers and leaders. Stop abusing Thai women by chaining them to this shameful way of life. Okay, I guess I can get off the soapbox now...
