You don't say what state this is in so you can't get an answer that is specific to your state. I would call your state adult protective services and I would also call the state department of mental health. Who ever regulates the care of the mentally handicapped. Believe me there is an agency in your state that does that. If you can't find it in the phone book call an agency that deals with the developmentally delayed and they should be able to let you know. He might have to be removed from his mother and put in a group home for the developmentally delayed. I hope that you find help for him. Worse come to Worse you can always call the police and they would have to report it to the correct department.

What you describe sounds more like neglect then abuse. If there was real abuse (example physical) then the local authorities could be called.
Is the individual under some federally funded program such as Medicaid or Medicare?
Many States have Disability Services that will provide a PCA (Personal Care Assistant) to come into the home and assist and individual with their personal care.
My guess is mom does not feel comfortable bathing her adult son, so it is not getting done.
I would get on the phone with Child Welfare and ask to speak to a Supervisor. Find out where in your State you can address your questions. The mom needs to be visited by a Social Worker to make an assessment of the home situation.
If mom is not properly able to care for her son, perhaps the answer is to move him into a group home.

Better yet, write the circumstances in a note to your Governor. As an employee for the State, this gets tremendous action--
The Gov's Office will involve the appropriate people.

In California there are a couple of avenues. I would think other states would have something similar. Instead of Children's Protective Services, there is also Dependent Adult Protective Services. It isn't as good as Children's Protective Services, but it is worth a try. The other is a program for people who are developmentally delayed. These people would help those in this category. In California they have to apply for it. It is county based and called a Regional Center. However, even if they are unable to intervene, they may know what to do. Do make a referral to one of these programs. You sound like a caring person and someone needs to step in to protect this young man.

Call your local ADULT PROTECTIVE SERVICES AGENCY and tell them the story and who these people are...you will be able to do this without the family finding out who reported them..Then the A P S worker will investigate the situation and take the proper, state mandated actions.
And by the way thank you for looking out for people who usually cannot look out for themselves.

Call the mother or send a note to the mom. If she refuses or you don't see change report this to the principal.
