Yes, and it had to withstand extensive challenge in order to be called a theory. Scientists do not use the term theory lightly.

That shows the scientists are open-minded enough to know that however good an idea is, and however good the idea is at predicting the behaviour of natural phenomena, that it still may not be the entire explanation of how the a particular aspect of the universe works. Too bad many religions do not have the same open-mindedness.
A theory may be useful, but remains a theory until conclusively proven.

Yes, but someday as new discoveries are made, this theory may be completed with new information and become more complex. Ultimately (if Earth will live enough) it will have this form (please assure you are familiar with latest in string theory before proceeding):

Yes, correct. A theory is used to explain things until it is better understood. Then the theory is updated or discarded. Think of it as a working model that tries to explain "HOW".
If you want "WHY", then you need to look within or speak with a spiritual leader.

That definition does not rule in the Theory of Evolution. When you look, evolution's theory has little in the way of "groups of facts or phenomena", and "repeated testing" always brings different results, most of which are inconclusive.
