Sometimes it's on the back of the license. But you don't sign it until the JP, or Minister completes your ceremony. Then you will sign your Marriage Certificate along with the person who married you. After that, you take your Marriage Certificate (not the license), to the Social Security office and have your SS changed. Then you take your documents over to the DMV and have your drivers license change. Then you will change your utility bills by providing them a copy of your Marriage Certificate. Don't forget your bank accounts as well.

You need to have a certified copy of the actual marriage license from the state (not the foo foo one you put in the frame...the actual legal document). That usually takes a week or so after the wedding date to obtain a copy. After that, you take it to the social security office and fill out their paperwork...wait a couple weeks for your SS Card. Then after that, to change your drivers license...you need to either have your new SS card or your marriage document and a piece of certified mail (like a bill or something) with your new name on it to get your new license...and look pretty cause they make a whole new one...pic and all.

You CAN'T change your name until AFTER the wedding.
And you must still go by your Maiden Name until you receive a new social security card.
Get a "Name Change Kit". It costs about $35.00 but it comes with ALL the forms you need and a CD to fill all the forms for you.
First you need to change your Social Security card, then you need your new SS card to go to the DMV, Passport Office, Bank, Insurance etc.
And you can do this anytime AFTER the wedding providing your marriage certificate is filed in the town your are married in - usuall 2 weeks after your wedding date.

in most places, the marriage itself functions as a legal/official name change if you choose to take his name. there are no other forms. you just start using his last name instead of your own (or hyphenated names, or whatever).
you will have to change your name with the social security administration, on your driver's license and passport, with the IRS, and on any bank/credit card accounts, gym memberships, magazine subscriptions, etc.
for most name changes, you just need to give them a copy of your marriage certificate (which you'll get after the wedding) and let them know what name you'll be using.

After your ceremony the preacher will have documents for you to sign, and your witness to sign also. This will be sent to the court house in the city you live in and filed, after all papers are in they will send you an original copy of your marriage license. You use your current given names on the certificate.
The marriage license has nothing to do with name change.
If the bride is going to adopt the groom's surname,
she sends her applications into each organization.
Driver's Licence, Bank & Credit Cards, Insurance, etc.
They will ask for proof of the marriage license.

You don't change your name until you are married. Then you use your marriage certificate as proof. You will need to go to Social Security and change your name with them as well as on your state's driver's license and any other Legal accounts, such as bank, etc.

I am in Ga and I went through the social security office. You will need to have your marriage license. That is now your new name. It is very easy.
