What you need varies from state to state. I just recently passed my lisence test. For a permit though, you need a sheet from your school saying that you're enrolled in school. Just go to the counseling center or main office and ask for your "proof of enrollment." Tell them you need it for the DMV.
When you go into the DMV you'll need the thing from your school, plus a checkbook to write out a check for fees, proof of vehicle insurance, original birth certificate, and original social securtiy card.
They will then have you take a short test and if you pass then they will have your picture taken and then give you your permit. Then you are free to go. Good luck. : )
