Or is it okay to go straight from the fridge to the cooled wort?

i'm of the school of thought that any radical environmental change isn't good for your yeast. you should remove it from the refrigerator and allow it to return to pitching temperature before you pitch. If it's a "smack pack" just smack it and let it inflate in your brewing area.

Pitch away. I never had any problems pitching chilled yeast as long as the wort is cooled. God, I miss brewing!

Yes, it's always a good idea to bring your liquid yeast up to room temperature before pitching. You'll notice that it also says this on the vial.
