He was 65.He had mild MR for 3 years and managing with pills. He was attending his wife in a hospital for 1 month climbing stairs and walking long distances.Several times he got slight pain in the chest at different times for 4 days and finally admitted in hospital for severe chest pain and wheezing. After 3 hours he felt like normal. They took ECG,echoCG and prescribed new tablets. He stopped the old pills and started the new.In the evening he felt uneasy and asked for Deriffillin injection and got it.After an hour he got severe pain in the chest and collapsed in 5 minutes. My questions are,
1.Whether he should have undergone a surgery earlier?
2.Whether he should have avoided exertion?
3.Whether he would have been saved,if admitted in a good hospital?
4.Whether he died due all these resons?

He died of all three reasons as I can see.
He should have undergone earlier surgery, because unnecessary delay can results in complications and inoperable too.
In practical life it's difficult to avoid strain, any way there was a total blowout of valve in the hospital.
He should have been in experienced hand.
Please have your wife treated.
Hope answered your question.

I'd say Loving_Heart is right.

I had MR, due to infection in one case, heart strain 2 years later. I recovered from both with rest followed by careful very gradual exercise. I never exercise to the point of being at all out of breath. I had to start at a 94 heart rate, 1.5 miles an hour on my treadmill for only 10 minutes. When my heart rate went up, it meant fatigue, so slow down and rest. It took many months, but I got so I could walk stairs, and do treadmill at 3.5 mph, and 10% incline for 30 minutes.

WIth that problem, rest is the first part of the cure, when possible.
