In the "Peter Pan" prequel called "Peter and the Star Catchers," his name was Black Stache. In this story, Peter cuts off his hand when Captain Black tries to steal the treasure that he had found.
In "Captain Hook: The Adventures of a Notorious Youth" his name is James Matthew.
In the movie Hook, his name is actually given as James Hook.
Based on these stories, I think it would be safe to say that his birth name is James Matthew. He adopted the nickname Black Stache when he became a pirate captain. And then he changed his name to Captain James Hook once he lost his hand. I don't think that is his "official" background though since just about all the stories were written by different people.
