Usually if he doesn't have gynecomastia, the breast will be large due to fat instead of breast tissue. In this case, he could get liposuction, but would probably be told to just lose weight by his doctor.

Lipo would probably be the first option, but I think removing some skin and cosmetically repositioning the nipple would be possible.
