There are probably many causes to what is hapenning to your piercing, without seeing it I can't really tell, but here goes.
1- What gauge is the hoop? If it is anything smaller then 12 gauge is a no-no, they can cause the "cheese-cutter" effect on the piercing.
2- The quality of the jewelry, I would recommend changing the hoop for a titanium curved 12gauge bar, or PTFE which both are bio-compatible and implant graded materials. When you do the jewelry change, go to a professional.
3- Moving the jewelry during the healing process is a big no-no too, the crust deposits on the jewelry (dried white cells and lymph) might go inside the hole which causes even more trauma.
4- Nipple piercings are very hard to heal, they are constantly irritated (just wearing a shirt, bra, sleeping on it, hitting it by mistakes...). Even a healthy and healed piercing can act up anytime.
5- Piercings can also act up before you have a cold or a flu, during periods (if you're a girl), stress...
6- Soap is also pretty bad, you body might not like it at all. I recommend you buy sea salt, and give you piercing sea salt soaks. Here is the recipe:
8oz of hot water
1\2 a tea spoon of sea salt
mix it together and let it cool then apply to piercing in a shooter cup for 5 minutes twice a day for 2 weeks.
Do not use rubbing alcohol, hydrogen peroxidy... They will kill good and bad bacteria, white blood cells which are very important for the healing and it will dehydrate the piercing\wound which will cause it not to heal...
*If you can taste the salt in the water, this means it is to strong.*

By the way, you can breast feed with a nipple piercing, but it might cause Mastitis althought the risks are very, very low, and Mastitis may be caused by a small cut aswell.

I think it matters on how big your bazookas are and how big the nipple is my friend had hers done and she was stacked so they were really pushing up on her clothes and of course her nipples were always hard so that didnt help either. Long story short her nipples were really big but also they started to get infected due to the constant rubbing of her clothes she had to remove them so they would heal. I told her to take my advice of going topless for some time so they could heal but she never listened oh well after a few weeks they were back to normal and she left them alone although her nipples still stayed hard most of the time maybe she jacked them up when they inserted the pin

well sometimes it takes 6-8 months even up to a year, depends on your body. to fully heal. ive had mine done for about 6 months, and they still hurt sometimes. it should be fine, just keep cleaning it, summers eve will work good, just make sure its unscented!!! (if its antibact. i dont know if it is) also every once and awhile mix up some sea salt with warm water, and soak over the nipple ( using a dixie cup is what i recommend, cause it fits pretty well, and just hold it over the nipple and let soak) for about 5 min.
also never use hydrogen peroxide ever!!!

u shouldnt be cleaning it with that soap first off always use a plain antibacterial non scent white soap. and sometimes a pericing can change randomly like that but it could be its getting use to the new water and def stop washing it with that soap you wouldnt believe what a difference it can make. also concider switching to bars they are better for piercing such as nipples and less irritation trust me on that! best of luck!

You probably have an infection. The best (and in my view most sensible) option is to remove the piercing and let it heal up. It is difficult to eradicate an infection when a foreign body (ie the piercing) is still in place.

They sound infected. You need to "always" clean them with anti-bacteria soap and hydrogen peroxide, often! ;-)
