Assuming you're based in London, try looking on the main website:

www.alzheimers.org.uk

You should be able to find a local branch, where I think someone would be able to assist you.

My mum cares for my Grandmother who was diagnosed with Alzheimer's in 1993 and she has enduring power of attorney as my Gran is in no way capable now of conducting even the simplest conversation. I can't remember how she went about it, but she is a member of her local branch and gets phonecalls frequently from people such as yourself. She can't help you as she lives in Cornwall, but it will be your local branch that can. Most Alzheimer's Society volunteers do it because it affects them personally so they know exactly what to do and how to go about it.
