Those symptoms alone cannot diagnose heart conditions like those you mentioned. If he's had a hole in his heart all along (which would be the case, they don't just 'appear') and he's been fine, unless it's getting bigger or his body is no longer compensating, then there's no reason to worry. Some people go their whole lives like that and never have a problem.
I would think that a problem with his valves would cause more issues than just light headedness sitting or standing. However, they could be the first signs of a more serious issue.
There has to be more to it than just what you've described for a doctor to make that diagnoises.

If that is the problem: many many people do just fine, and their lives are so much better after surgery
If it isn't the problem: the other things that could cause that, barring some type of heart failure or hypertrophy (which I doubt), are more or less minor issues

A lot of people have had these heart problems fixed without major open heart surgery. I know a hole in the heart can be fixed by going through a vein in the groin area that travels to the heart and you are awake or can be for the whole thing. I dont know if all doctors do this, but I would find one that does it lowers a lot of risks. You can do a search online about it Ive done my own research about it there is a lot of info.

If this has been going on for quite a while so that short-term effects, such as dehydration, can be ruled out, then he needs to be checked out by a doctor. There is a high probability that your mom's health insurance will cover it, especially if the doctor is affiliated with the same hospital where your mom is an RN.

Do not worry. Many people have bad valves in their hearts.
Many problems with the heart are fixable. Good thing.
You need to talk to your Mom more about what is going on.
Your brother may not even have a heart problem...dehydration can cause light headedness, so can a virus.
