Avoid plastic bottles which will leech chemicals into your water over time. The best bottle is a stainless steel bottle. Even better is a stainless steel bottle with a brass lining (as brass has anti-bacterial properties).

I actually have gotten and really like the Stainless Steel bottles made by Klean Kanteen. I have several different sizes I have 12 oz that has a sippy adapter, I found if I remove their Avent sippy part the Nuk orthodontic nipple from my son's 5oz Gerber bottle actually fits in there perfectly. So the 12 oz ones get used as a bottle for my 4 month old son, the 18oz ones I use with the sippy adapter for my 2 year old. They do leak a little out the vent hole if there is pressure built up in the bottle, but I just wipe the top with a rag if I see it has leaked. I have the 27oz bottle I use with a loop cap (they also have a sport cap, and a flat cap). These bottles are all sized to fit in the cupholder in my truck as well as in bike water bottle cages. I find they wash up nicely in my dishwasher too.
I figure that these are all long term investments, and will grow with our family by just using different caps. They have them at Amazon.com or elsewhere on the web too. Hope this helps.

Nalgene makes all size of sturdy water bottles. There are a lot of imitations but get the Nalgene brand bottles; they're the best and made in the USA. Any sporting goods store will carry them or go to their website.

Try Wal-mart , they seem to have a huge selection, including the thermal water bottles. My second place to go would be a sporting goods store.

I've been searching endlessly for the same thing that ALSO
*fits in my cup-holders in my car
*is shaped so it is possible to wash well in the dishwasher and/or by hand
*doesn't have rubbery parts for my kids to chew on and destroy
*is durable
I'd love recommendations that have been successfully used by families.

Any bicycle shop will have a variety of inexpensive and sturdy water bottles in a very convenient size.
An outdoor store will have a greater selection, but many will be inconveniently shaped.

years ago soda came in deposit bottles and so did milk...we would just take it back to the store when we shopped and get our deposit back....
in stores like trader joe's they have glass milke bottles with reuseable lids...there is a deposit but jus tkeep the bottle...it keep the water great and your saving the environment...
I do think a boycott on soda to get the companies to get smart and out the plastic business...its bad enough to drink things without the plastic and the deadlier side effects....so if we, everyone on the answers site would boycott pepsi & coke do you think they would go back to glass? Let all email pepsi & coke and tell them that at Answers we like in glass...and lets see what they do...let me know what you think? we can go world wide....
