Bowel perforation, but this is rare. Puncturing a bowel can cause fecal matter to leak into your body, causing major sepsis. Puncturing a bowel can also result in massive internal bleeding. Both are definitely not good for you.
Plenty of people enjoy anal sex safely and without complications. Safer anal sex requires that the penetrative partner wears a condom and applies a LOT of water-based lube to the receptive partner's anus.
You can pick up infections during unprotected anal sex, though. HIV is the most important to consider because it leads to AIDS. But you can also get HPV anally, and that can lead to anal cancer, and like other cancers, it could kill you, too.
ADDENDUM FOR IRISHGIRL: You make a good point about the unlikehood of bowel perforation during regular anal sex. However, there are people who enjoy engaging in alternative sexual practices (usually safely), and for them perforation could be an issue depending on the nature of the item penetrating the rectum.

There seems to be a homophobic undertone to this question.
Technically, any kind of sex can kill you. Yes, the lining of the rectum is thinner than the lining of the vagina and that tearing can occur. Tearing can also occur with vaginal sex leading to infections, abscesses/septicemia, and death.
Any time you have anal OR vaginal sex it is VERY important to use a water-based lubricant (regardless of whether you are using condoms or not). This will help to prevent the tearing of the vaginal or anal mucosa and therefore prevent infections (including HIV).
The comment earlier from the "RN" I strongly disagree with. I know people (both straight and gay) who have been practicing anal sex for decades and a few couples who have been "doing it" for longer than I have been breathing.
Yes, it is possible for the anal sphincter to become stretched (it happens a few times a day for most everyone in the world when they defecate/poop). I challenge the RN to provide scientific peer reviewed data that shows that anal sex invariably leads to bowel incontinence.

*sigh* AIDS? It is easier to transmit during anal sex because the tissue tears easily. A perforation should NOT be an issue unless your partner's penis is barbed. Anal sex is very safe and clean when done properly. Trust me on this; I know. BJC, thank you for educating these people. Your sphincter will not be anymore stretched than if you defecate.

Anal sex causes your sphincter muscle to become loose, this is what holds your fecal material in place. In other words don't do it, cause eventually you will not be able to stop yourself from having bowel movements and they will come anytime. This goes for male and females.
And if your partner has a disease then he will transmit it to you. Some STD cannot be cured. Women do not have the same symptoms that men do. Women sometimes don't know they have an STD so it remains untreated and will cause the women to become sterile.
I don't know if you know who Al Capone is but read about him , he died from an untreated STD.

You **** is full of bugs, both hormelss and harmfull, During anal sex, you pick up all kind of bugs. Once I asked a gay man, how do you clean your penis after sex, he said, the ejeculation cleans it. WRONG it was surprising since the man was well educated. After anal sex you need to clean with soap and water, how many do. so you end up with infections and if bad one, you can die or get HIV and that will be the END.
