A nutritious salad is always a good substitute. A diabetic should avoid the carbs in the pasta and the fats in the cheese. (as anyone interested in good health should),

There is pasta by a brand called Dreamfields... and they have a macaroni:

Also, try any whole grain:
Myth #5 If you have diabetes, you should only eat small amounts of starchy foods, such as bread, potatoes and pasta.
Starchy foods are part of a healthy meal plan. What is important is the portion size. Whole grain breads, cereals, pasta, rice and starchy vegetables like potatoes, yams, peas and corn can be included in your meals and snacks. The key is portions. For most people with diabetes, having 3-4 servings of carbohydrate-containing foods is about right. Whole grain starchy foods are also a good source of fiber, which helps keep your gut healthy.

There is no reason not to eat pasta in moderation.
People with diabetes can eat every food on this planet.

For a low calorie/very low carb mac& cheese try using a spaghetti squash mixed with cheese sauce.

I like it best with butter, cinnamon ,nutmeg and a bit of brown sugar.

As long as you cover it and eat just a cup, you'll be okay. Whole wheat pasta is generally lower in carbs than the white stuff, so go that route.
Low fat cheese is also a good choice...just make sure you count the carbs and eat only one serving.

i don't know where u live but i live in illinois and it takes me and my mom and hour or so to drivew to this store. they have pasta tha's only 40 carbs in 2 cups. and usually it's 40 carbs in 1/4 of a cup that store is so helpful, but it's only in illinois, and they have stuff with low carbs. oh and by the way mac and cheese have 40 carbs in 2 tablespoons so diabetics aren't supposed to eat it.

you can eat it but very little bit maybe i fyou really like it take 1 cup and in one week or maybe two weeks. it is that startch is sugar and that is what it turns into that is why they tell you that. actually all you eat turns to surgar one way or another. you have to be careful and juggle your diet. it can be once in awhile you can eat what you shouldn't but then go back to normal for awhile depending how bad off you are if you are very bad then you will need to obstain from this awhile longer till you get thenubemrs down. remember its what you eat that messes you up and are you ready for consequences. just keep cool and move on it will come down.

There is whole wheat pasta on the market, check the label, sometimes they list the glycemic index on the box.
