he does check his blood sugar level a few times a day, he is kind of reckless, but I can't help it, I can't force him into going to see his doctor more often.

this can't be that bad, he's had it many times, there has never been a problem (as yet).

He probably won't smell it ... at least, I've never noticed it on myself. Others, however, can smell it. It smells of acetone (pear drops).

Usually, the only time I know I have keto-acidosis is when I check my blood ... several times a day, as it happens, and my result shows that I have a high blood sugar. I am then able to check, with a different type of test strip (ß-Ketone test strips), to see how high my ketone levels are.

If blood sugar is raised, this is what, invariably, happens. One of the dangers is that raised blood sugar levels leads to complications. As has already been stated, however, it can be fatal if not treated urgently.

I had a friend who was a type one diabetic and she was compulsive about checking her blood sugar. She never let it get high. And she often had ketonic breath. I never mentioned it to her because I figured there was nothing she could do about it. I don't think she realized it, but I don't know for sure. Not really an answer to your question.

If you mean ketonic breath then being able to smell it is the least of his worries. It is a sign of ketoacidosis which means he probably isn't controlling his diabetes properly. Get him checked out by a doctor asap as this is often fatal.

You are I assume talking about diabetic keto-acidosis. Diabetics make become ketotic and indeed go slowly into coma, without realising what is going on.
