Tarpaulin - to shield from sun and catch rain water to drink and if you put salt water and let it soak through - you can survive on the drips.
Flint - fire is essence to protection against wild beasties and raw fish is ok for sushi but not every meal.
Knife - can be used for building, protection, cooking.
Harmonica - need a little music to keep me sane.
Luxury item - BIG bottle of tequila - ya never know what might be on that island with you....you might get lucky!
