It's going to vary plan by plan, and state by state.
MOST plans cover 50% up to $1800. So dad pays half, and if it's more than $3600, he pays more than half.

It depends upon the benefit package that your dad's employer set up.
There is no set amount that any one insurer would pay - its based on the amount negotiated between the employer and the insurance company. (Some employers prefer to shift most of the cost to the employees, to save on premiums. Other employers may choose to have most of the cost covered by the benefit plan, as a perk to the employees.)

Even as a Nationwide agent... i'm going to say ditto to every one else.....
Every policy is different, have your dad call his agent, or call Customer Service if it's a group policy. They'll be able to tell him exactly within 2 minutes.

you'd have to call them to find out because each policy MIGHT be different but i work for Wellpoint (Anthem) and we will pay $1500.00 max if you carry the dental benefit.
